Facility Evaluation Report

Licensing Program Analyst (LPA) David Roman conducted an unannounced case management visit to follow up on an incident report involving AWOL's. LPA identified himself and was granted entry by Jose Navarro, Manager. LPA stated the purpose of the visit and reviewed the basic elements of this visit with the Manager.

The Department conducted an investigation for an incident which transpired at the social rehabilitation facility on May 12 and 18, 2026. It was confirmed that client (C1) had AWOL'd from a medical facility. It was also confirmed that the client (C2) had AWOL'd from another medical facility. Program staff reported that in both separate instances clients were transported to receive medical services and at the end of their treatment clients chose to voluntarily leave the program.

During today’s visit, LPA conducted an interview with the Manager, Jose Navarro and conducted a record review, regarding the incident for additional information, as well as a walk through throughout the facility. Based on the information obtained during the visit the facility staff conducted their due diligence, deficiencies were not observed or cited during this CM visit.

The report was discussed and an exit interview was conducted with Manager, Jose Miguel Navarro, to whom a copy of this report along with Licensee/Appeal Rights were provided at the conclusion of the visit. Their signature below confirms the receipt of their rights and this report.
