Facility Evaluation Report
Licensing Program Analyst (LPA) David Roman conducted an unannounced Case Management Inspection/ Health and Safety visit. LPA D. Roman identified himself to Licensee, Raquel Preece and discussed the purpose of the visit.
LPA D. Roman conducted a resident interview, staff interview, and record review, which identified the resident was having self harming bxs on 7/10 due to being set up during a date in which the resident was having social media communications that led to a potential scam. The resident also entered the local Walmart on 07/11 however, has a one month trespassing citation and was successfully redirected back to the facility. Records reviewed also identified residents medication had been changed and are slowly increasing to manage anxiety and aggressive bxs.
LPA D. Roman along with facility staff toured the interior and exterior of the facility and inspected bedrooms and bathrooms. The facility was sanitary, and in good repair. Pathways were free of obstruction and slip hazards. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and resident activities.
