Facility Evaluation Report
Licensing Program Analyst (LPA) David Roman conducted an unannounced case management visit to follow up on an unusual incident that occurred during the weekend. LPA D. Roman identified himself and discussed the purpose of the visit with the Licensee.
Today’s visit was in response to an Unusual Incident Report regarding a resident that fell during a community pass. The resident contacted 911 via phone to ask for assistance. Law enforcement arrived to the scene and transported the client to the hospital. The client suffered a superficial scratch on his arm and was advised to avoid the heat and to ask the facility for rides. Licensee, Raquel Preece, House Manager, Giovanna Lira, and Administrator, Derek Hampton were notified by PD. LPA D. Roman conducted a tour of the interior and exterior of the facility. LPA interviewed staff and residents for additional information about the incident. Licensee reported that the client has recently had a change of MHS medication, and behaviors have improved. No deficiencies were cited during today’s visit.
An exit interview was conducted with Licensee, Raquel Preece and a copy of this report along with Licensee/Appeal Rights (LIC 9058 01/16) were provided. Their signature below acknowledges the receipt of this report and their rights.
