The inspector’s account

Licensing Program Analyst (LPA) David Roman conducted an unannounced case management visit to follow up on an unusual incident that occurred during the weekend. LPA D. Roman identified himself and discussed the purpose of the visit with the Licensee.

Today’s visit was in response to an Unusual Incident Reports regarding a resident which exhibited explosive aggressive behaviors towards other residents and staff. The resident also attempted to steal merchandise from Walmart. Law enforcement was contacted and the resident was placed on a 5150. The incident occurred on May 22, 2026 and was subsequently reported to Community Care Licensing. Licensee, Raquel Preece, House Manager, Giovanna Lira, and DSP, Darleen Lopez were present during the case management/health and safety visit. Along with facility staff, LPA D. Roman conducted a tour of the interior and exterior of the facility. LPA interviewed staff and residents for additional information about the incident.

Licensee reported that the client has recently had a change of MHS medication, BCBA interventionist assist with monitoring behaviors, conflict resolution, and behavior interventions. No deficiencies were cited during today’s visit.

An exit interview was conducted with Licensee, Raquel Preece and a copy of this report along with Licensee/Appeal Rights (LIC 9058 01/16) were provided. Their signature below acknowledges the receipt of this report and their rights.
