This is an official report of an unannounced visit/investigation of a complaint received in our office on

04/14/2025 and conducted by Evaluator Christopher Arnhold

COMPLAINT CONTROL NUMBER: 21-AS-20250414081654

At approximately 11:15AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct an investigation into the above allegation. LPA met with Administrator Jennifer Hall, reviewed records and interviewed staff. Based on records reviewed and interviews conducted, LPA did not find evidence to support the allegation listed above. The Humboldt County Sheriff's Department conducted an investigation into the matter and did not find evidence to support the allegation. Resident was interviewed and provided a different scenario that involved things that are not based in reality. Resident was checked for injury and there was no indication of any sexual abuse or injury found.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.
