Facility Evaluation Report
At approximately 9:45AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct a case management visit in regards to several medication errors. LPA met with Administrator Taleisha Rose and House manager Haleigh Nevergall and reviewed records. Facility has had 6 medication errors due to various reasons. LPA reviewed procedures for medication passes and found it to be suitable. LPA was informed all staff have received updated medication training from a registered nurse on July 27. LPA reviewed each incident and found facility addressed the issue with staff each time and updated procedures or terminated staff.
Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
This report was reviewed with Taleisha Rosa and Appeal rights were given.
