This is an official report of an unannounced visit/investigation of a complaint received in our office on

05/23/2025 and conducted by Evaluator Christopher Arnhold

COMPLAINT CONTROL NUMBER: 21-AS-20250523081808

At approximately 2:00PM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to deliver investigative findings from an investigation into the above allegations. LPA met with Administrator Heather Craven. Based on interviews conducted and records reviewed, LPA did not find evidence to support the above allegations. LPA interviewed all staff working at the facility and learned no instances of verbal or physicial abuse took place and staff enjoy working with their co-workers.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated.
