Facility Evaluation Report

At approximately 11:15AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit in regards to an eviction notice given to the facility. LPA met with Licensee Tina Rosa and House Manager Kim Vargas and reviewed records. On June 4, 2026, the Department was notified that the Licensee received a 60 day eviction notice for the facility. Licensee told LPA that they are currently searching for a new location to move the facility to.

Upon arrived, LPA observed a for sale sign posted on the property. LPA spoke with Licensee to request documents regarding the eviction. Licensee informed LPA they toured a new home earlier this day and will be submitting a change of location application as soon as they finalize their agreement.
