Facility Evaluation Report

At approximately 8:15AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct a complaint investigation. During this visit, LPA followed up on two incident reports submitted by the facility on 12/12/2025. The first report was in regards to client (C1) eloping from the facility on 12/11/2025. C1 was reported to have made comments to staff that they were homesick and wanted to return to their previous placement. C1 was observed by staff in their room at approximately 4:00PM on 12/11/2025, during the hourly room check. At approximately 5:00PM, staff observed C1's outside door was open and C1 was not present. Facility contacted the conservator and law enforcement regarding the elopement. C1 was located by law enforcement and returned to the facility at approximately 11:45PM. On 12/12/2025, C1 became aggressive towards staff and law enforcement came and removed C1.

During this visit, LPA reviewed C1's records and observed C1 is able to be in the community without supervision.

Facility followed their policies and regulation.
