Facility Evaluation Report
At approximately 8:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to continue a Required-1 Year inspection. LPA met with Nursing Supervisor Melissa Fisher and explained the purpose of the visit. At approximately 8:15AM, LPA reviewed 4 of 4 resident files and 5 staff files. All resident files contained the required documentation. Reappraisals were conducted within the last 12 months and daily progress notes are created for each client. Documentation of a physician visit within the last 12 months was present. Staff files reviewed contained evidence of completed required training and First Aid/CPR certification was current. At approximately 8:45AM, LPA conducted a review of medications. Medication is locked and not accessible. During this inspection LPA followed up on an incident report submitted by the facility on 06/30/2025. The incident was in regards to a medication error. Staff gave a resident twice the amount of medication ordered by the Physician. Administrator ensured retraining for all medication personnel is being conducted to be completed by July 31. LPA cleared the citation during visit.
