Facility Evaluation Report

At approximately 11:45AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit in regards to a medication error that occurred on 06/15/2023. LPA met with Program Director Wes Patterson. Based on interviews conducted, the error occurred due to a staff forgetting to assist the resident at the proper time. Facility has updated their medication procedures and set up reminders on multiple devices to ensure client receives their medication at the proper time.

Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.

This report was reviewed with Wes Patterson and Appeal rights were given.
