The inspector’s account

At approximately 8:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit in regards to a report of suspected abuse. LPA met with Licensee Colleen Rasmussen and reviewed records. On 01/12/2026, Licensee reported to Licensing that a resident, R1, informed them that staff, S1, kicked and verbally abused them the previous night. Licensee reported the information to Law Enforcement. LPA received copies of documents.
