Facility Evaluation Report

At approximately 09:15AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct a case management visit in regards to a Special Incident Report submitted by the facility. LPA met with Administrator Sara Anderson and reviewed records. The incident occurred on 09/23/2025, when a resident, R1, returned to the facility at approximately 11:00AM. Facility staff were not aware R1 left building. Facility conducted an internal investigation and found the last staff contact with R1 occurred at approximately 9:30AM that day. R1's normal behavior is to walk around the facility. LPA reviewed resident file and observed there was no indication of prior exit seeking behaviors. Facility immediately updated resident assessment and contacted the doctor to review medications. All notifications were made and R1 was assessed and no injuries were found.
