Facility Evaluation Report

At approximately 8:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit in regards to an incident report submitted by the facility on 08/14/2025. LPA met with Executive Director Sara Anderson and reviewed records. On 08/12/2025, medication technician accidentally took an oxycodone from its package before realizing it had already been administered. The pill was set aside for disposal and then misplaced. Executive Director conducted an investigation and determined the medication was inadvertently disposed of. Director has reviewed policies and procedures and will submit an updated version to CCLD for approval. All medication staff are in the process of retraining and documentation of completed training was provided during this visit.
