Facility Evaluation Report

At approximately 2:00PM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a case management visit in regards to an incident report submitted to CCL on 04/29/2022. LPA met with Executive Director Tiffany Blake. The incident involved a medication going missing. Staff learned of the missing medication when a new medication was about to be started. A pill count found the bottle was missing 1 tablet. An investigation was conducted and the pill did not turn up. A consulting company conducted an audit on 05/12/2022 and provided training to medication personnel. Director
