Facility Evaluation Report
On 2.25.26 Licensing Program Manager (LPM) Laura Munoz, Licensing Program Analyst (LPA) Lavinia Muscan, BCBA Nicole Reed, and ALTA Regional Representative DeDe Peters met with Director of Residential services Shoa Axum and Administrator Jesus Ramirez Orozco and and explained the purpose of the visit.
The facility submitted an exception request to use a locking mechanism on an exterior gate of the facility for R1. The gate would be removable and used to assist staff when R1 was having elopement behaviors.
Department discussed with the facility and behavior specialist regarding the plan on using the locking mechanism.
The department has advised the facility to submit the following documents and information to support the exception:
Facility was advised that once all the documentation is submitted, the department will review and advise if exception is approved.
Facility is advised that the locking mechanism shall not be used until an exception is approved by the department.
