Facility Evaluation Report

Plan of correction visit to clear the deficiency from the 10/11/21 complaint visit. Title 22 - 87303(a) plan of correction was due on 10/18/21. As of 10/19/21, the plan of correction has not been submitted for review and clearance. Civil penalty of $100 was issued and shall accrue at $100 per day until the deficiency is corrected.
