LPA Muscan conducted a case management on 06/01/2026 to verify facility is closed. LPA Muscan toured the facility with Administrator Mary Hensley to confirm closure. Facility walk through revealed there are no residents in care. Last resident moved out on 02/28/2026. Residents and their families were notified of the closure, as well as CCLD.
No deficiencies cited during today’s inspection.
No further action is needed. Exit interview conducted.
