Facility Evaluation Report
On 02/10/26, around 01:30 PM, Licensing Program Manager (LPM) Troy Ordonez, Licensing Program Manager (LPM) Laura Munoz, and Licensing Program Analyst (LPA) Lavinia Muscan were present for a Non-compliance Conference with Licensee/ Administrator Jose Santos, which was held in-person in the office.
This conference does not in any manner excuse past problems or resolve the Department’s case against the licensee if the problems are not corrected. The Non-Compliance Conference may be the last step prior to initiating administrative action following unsuccessful attempts by the Department to gain compliance.
Since 2020, the facility has been issued one (1) A citations, and assessed 1 civil penalty in the amount of $500. Licensee was cited for Criminal Record Exemption 87356(a)(1).
Training, policies, management oversight, ensuring care staff is aware and familiar that no individual who has been convicted of a crime for which registration as a Registered Sex Offender (RSO) is required, is residing at the facility or has presence/contact with clients/ residents in any manner. Implement internal audits and increased administrative oversight.
The licensee/administrator was issued citations which are documented on the LIC809-D.
The licensee agreed with the drafted non-compliance plan as outlined in LIC 9111.An exit interview was conducted, and a copy of this report was provided to the licensee. Appeal rights provided.
