Facility Evaluation Report
Licensing Program Analyst (LPA) Lavinia Muscan arrived on February 23, 2025 to conduct the annual inspection.
On this date, LPA observed approximately 4 residents in the activity room doing activities. Of the residents present, one (1) of them is not a resident of the care home but is here for the day program activities that are conducted on site.
LPA observed that the residents present are all participating in the activities and appear to be happy. LPA observed that the space being utilized for the activities is well kept and ample for the residents participating on this date.
LPA and staff toured the facility together to ensure the health and safety of residents in care. In the areas toured, there were no health or safety violations observed.
LPA requested a copy of the LIC 500, LIC610E and current liability insurance to be sent to the Department by end of the month.
No deficiencies cited. Exit interview conducted. A copy of this report was left at the facility.
