Facility Evaluation Report
On 07/07/2026, at 10:30 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Case Management - Annual Continuation to complete the Required 1 Year inspection started on 07/06/2026. Upon entry, the LPA explained the purpose of the visit to Caregi vers Maribel Saldevar and Joanna Magalong. The Administrator (ADM) Nucharee "Joy" Yokdang arrived at approximately 11:00 AM.
The LPA inspected the community and observed the required postings in the community. The fire extinguishers were last serviced on 06/25/2026. Carbon monoxide and smoke detectors were fully operational. The Emergency Disaster Plan was reviewed by the ADM on 02/15/2026. Emergency and disaster drills are conducted quarterly; the most recent was conducted on 06/01/2026.
The LPA observed that the centrally stored medication cabinets were locked, each resident's medication was stored in its originally received container, and staff dispense medications directly from the original co ntainer to residents. The first aid kit was observed to be complete.
The liability insurance expires on 09/30/2026. An administrator is on site more than the minimum of 20 hours a week to ensure proper business operations are being conducted.
The LPA reviewed community records, 5 resident records, and 5 staff records.
No citations were issued during the inspection.
