Facility Evaluation Report
On 7/31/2025, at 8:00 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to complete the Required 1 Year inspection that began on 6/30/2025. Upon entry, the LPA informed Caregiver Joanna Magalong of the purpose of the visit.
The LPA reinspected the interior of the facility with Caregiver Magalong while speaking with Licensee / Administrator Nucharee (Joy) Yokdang over the phone. They reviewed changes made to better store medications and cleaning products in accordance with the LPA's suggestions from the incomplete inspection on 6/30/2025.
The LPA reviewed facility and 5 staff records.
The Required 1 Year inspection is now complete.
No citations were issued during the inspection.
