This is an official report of an unannounced visit/investigation of a complaint received in our office on

06/08/2020 and conducted by Evaluator Jill Clancy-Czuleger

COMPLAINT CONTROL NUMBER: 15-AS-20200608105919

On 10/18/2022 starting at 01:05 pm, Licensing Program Analyst (LPA) J. Clancy-Czuleger arrived unannounced to conduct complaint investigation for the above allegation. LPA met with Supervisor, Marilou Ladaban and explained the purpose of the visit.

During the course of investigation, LPA obtained information, collected documents, and interviewed staff. The facility was keeping a handwritten account of the resident’s funds, and what the funds had been spent on.

Although the allegations may have happened or are valid, there are not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
