On 1/29/2026, at 10:00 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct a Plan of Correction (POC) visit. Upon arrival, the LPA stated the purpose of the visit to Caregiver Blessilda Luna and Licensee Donald Hay by phone.
