Facility Evaluation Report
On 08/12/2025 at 8:30 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct the Required Annual Inspection. Upon entry, the LPA stated the purpose of the visit to Caregiver Noli Barrozo. The Licensees Mylin Perdigue and Claro Villanueva arrived at approximately 9:30 AM.
The LPA inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. Temperature in the dining room was measured at 70.3 degrees Fahrenheit and the hot water in the kitchen was measured at 107.2 degrees Fahrenheit. The fire extinguisher was fully charged and last replaced 07/16/2025.
The carbon monoxide and smoke detectors were fully operational. The LPA observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations.
The LPA reviewed records of 5 residents. No citation was issued during the inspection.
The Required Annual Inspection was incomplete. The Department will return unannounced at another day and time to complete the inspection.
