On 04/15/2025 at 11:15 AM, Licensing Program Analysts (LPAs) Y. Brown and J. Sampair, arrived to conduct an unannounced pre-licensing inspection of the facility. LPAs met with caregiver Ashley Ingleton and spoke with him regarding the purpose of visit. Administrators Telesha Clarke and Timea Yost arrived at the facility at 11:30 AM.
During the inspection, the LPAs issued one (1) type B citation.
The exit interview was conducted and a copy provided.
