Facility Evaluation Report

On 06/17/2026 at 02:00 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct a case management visit to deliver an immediate exclusion letter. The LPA met with Caregiver Joy Fernandez and explained the purpose of the visit. The Administrator (ADM), Norberto Geronimo, was out of the facility.

During visit, the LPA hand delivered the immediate exclusion letter for Staff S1 to the Caregiver, who stated that S1 had resigned. The LPA communicated with the ADM via voice mail and text messaging. Through text messaging, the ADM confirmed that S1 is not currently employed at the facility and will be immediately removed from the facility's Guardian Roster.

No deficiencies were cited during this visit.
