Facility Evaluation Report
On 3/10/2025, at 1:45 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Required Annual Inspection of the facility. Upon arrival, LPA stated the purpose of the visit to Nurse Mark Talaro, RN. Administrator Paolo Paredes arrived at approximately 2:15 PM.
The LPA toured the interior and exterior of the facility. LPA inspected the kitchen, dining area, restrooms, community living spaces, bathrooms, resident rooms, and the grounds of the facility. The LPA observed adequate lighting for the comfort and safety of residents in all rooms. The temperature in the dining room was measured at 68 degrees Fahrenheit. The maximum hot water temperature was measured at 108 degrees Fahrenheit in the kitchen. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. Inside and outside areas are free of obstruction and no bodies of water. The LPA observed the required postings in the facility.
Carbon monoxide and smoke detectors were fully operational. Fire extinguisher was fully charged and last serviced on 2/5/2026. The Emergency Disaster Plan was reviewed within the past year; the most recent review was on 1/29/2026. Emergency, disaster, and fire drills were conducted on a monthly basis; the most recent was 2/11/2026. The first aid kit was observed to be complete. An administrator is on site more than the minimum of 20 hours a week to ensure that proper business operations are being conducted.
The LPA reviewed facility records, records of 5 staff members, and records of 5 residents.
