Facility Evaluation Report

On 4/7/2025, at 9:50 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced at the facility for a Case Management visit. Upon entering, the LPA informed Administrator Paolo Paredes of the purpose of the visit.

The LPA met with the Administrator concerning the deficiency, which was not providing notice and documentation to the Department within 30 days of hiring the new administrator, Paolo Paredes, (Refer to California Code of Regulations Section 87211(g)).

One Type-B citation was issued (refer to LIC 809-D).

An exit interview was conducted and a copy of this report provided.

Facility is subject to civil penalties if proof of corrected deficiency has not been sent to Community Care Licensing (CCL).
