The inspector’s account

On 04/21/2026, at 10:00 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Required Annual inspection of the facility. Upon arrival, the LPA stated the purpose of the visit to Staff S1. Administrator Maria Angelica Bonuan arrived at approximately 12:15 PM.

The LPA toured the interior and exterior of the facility. LPA inspected the kitchen, dining area, restrooms, community living spaces, bathrooms, client rooms, and the grounds of the facility. The LPA observed adequate lighting for the comfort and safety of clients in all rooms. The temperature in the kitchen was measured at 71.6 degrees Fahrenheit. The maximum hot water temperature was measured at 115.5 degrees Fahrenheit in the kitchen. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. The interior and exterior areas of the facility are free of obstruction. There are no bodies of water. The LPA observed the required postings in the facility.

Carbon monoxide and smoke detectors were fully operational and last tested by the staff on 04/11/2026.

Fire extinguisher was fully charged and replaced on 04/13/20 26. The Emergency Disaster Plan was reviewed within the past year; the most recent review was on 4/21/2026. E mergency, disaster, and fire drills were conducted on a monthly basis; the most recent was 04/11/2026. The first aid kit was observed to be complete. An administrator is on site more than the minimum of 20 hours a week to ensure that proper business operations are being conducted.
