Facility Evaluation Report
On 4/7/2025 at 10:30 AM, Licensing Program Analysts (LPAs) Y. Brown and James Sampair arrived unannounced to conduct the Required Annual Inspection of the facility. Upon arrival, the LPAs stated the purpose of the visit to Administrator (ADM) Connie Vigo.
The LPAs toured the interior and exterior of the facility with the ADM. The LPAs inspected the kitchen, dining area, restrooms, community living spaces, bathrooms, resident rooms, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. Temperature in the living room was 69.5 degrees Fahrenheit. Fire extinguisher was fully charged and last serviced on 03/12/2025. The hot water temperature in the kitchen was 105.1 degrees Fahrenheit. Emergency Disaster plan was observed and last updated on 10/01/2024 and the last fire drill was conducted on 03/11/2025. Carbon monoxide and smoke detectors were fully operational.
An administrator is on site more than the minimum of 20 hours a week to ensure that proper business operations are being conducted. The LPAs reviewed facility records, records of 5 staff members, and records of 5 residents.
