On 1/10/2024 at 9:00 AM, Licensing Program Analyst (LPA) J. Sampair arrived unannounced at facility to deliver an Employee Immediate Exclusion Letter concerning Caregiver C1. Upon entry, LPA stated the purpose of the visit to Licensee Tina Camaclang.
At approximately 9:10 AM, LPA delivered the C1 Employee Immediate Exclusion Letter to the Licensee.
Exit interview conducted and a copy of this report provided to Licensee via email.
