Facility Evaluation Report
On 9/19/2025, at 12:15 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced for this Case Management - Health Checks visit. Upon entry into the facility, the LPA stated the purpose of the visit to Licensee / Administrator (ADM) Albertina Camaclang.
LPA and ADM checked all of the perishable and nonperishable foods in the facility. One of the perishable and 55 of the nonperishable foods were expired and thrown out. A Type-A citation was issued for this deficiency (refer to LIC 809-D) . After throwing out the expired foods, the facility still had more than the required minimum of 2 days of perishable and 7 days of nonperishable food for the residents.
Deficiencies are cited from Title 22 California Code of Regulations (refer to LIC 809-D). Failure to submit proof of correction may result in additional civil penalty.
Exit interview conducted and a copy of the Appeal Rights, and this report provided.
