On 9/8/2025, at 1:45 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Case Management visit to deliver the amended report for the complaint #15-AS-20250805113016. U pon entry into the facility, the LPA identified himself and stated the purpose of the visit to House Manager Precilla Cabrena.
