Facility Evaluation Report
On 08/05/2025 at 01:20 PM, Licensing Program Analysts (LPAs) James Sampair and David Doidge arrived unannounced to conduct the Required Annual Inspection. Upon entry, LPAs stated the purpose of the visit to Licensee Carla Blaj.
LPAs toured the facility including but not limited to residents’ rooms, bathrooms, kitchen, common area and courtyard. LPAs observed lighting in all rooms are adequate for the comfort and safety of the residents. Hallway temperature was maintained at 70 degrees Fahrenheit. The hot water temperature was measured at 107.4 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2 days of perishable foods. Centrally stored medications, sharps and toxic are locked and inaccessible to residents in care.
The carbon monoxide and smoke detectors were fully operational. The LPAs observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy.
Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 03/17/2025. Emergency Disaster Plan was last reviewed on 07/11/2025 Emergency, disaster, and fire drill are conducted quarterly, last conducted on 07/11/2025. First aid kit was observed to be complete.
The LPAs reviewed four (4) residents and four (4) staff records; all were complete.
No citations were issued during the inspection.
