On 8/11/2025, at 3:15 PM, Licensing Program Analyst (LPA) James Sampair arrived announced to conduct a Case Management inspection to close facility. Upon entry into the facility, the LPA confirmed the purpose of the visit with Caregiver Tristan Almario.
The LPA toured the facility and verified that no clients were living in the facility.
