Facility Evaluation Report
On 05/05/2025, at 1:00 PM, Licensing Program Analysts (LPAs) Y. Brown and J. Sampair arrived unannounced to conduct the Required Annual Inspection. Upon entry, the LPAs stated the purpose of the visit to Caregiver Demarto Acupan . Administrator Anthony Oshinowo arrived at approximately 2:00 PM.
The LPAs inspected the interior and exterior of the facility, including the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. Water temperature in the restroom was measured at 105.1 degrees Fahrenheit. The fire extinguishers were fully charged and last replaced on 09/24/2024. The Emergency Disaster Plan was posted and last updated on 1/12/2025. The carbon monoxide and smoke detectors were fully operational.
An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. The LPA reviewed facility records, records of 3 staff members, and records of 5 residents.
