On 05/28/2024 at 1:45 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced for a case management visit to complete the Required Annual Inspection began on 5/21/2024. Upon entry, LPA disclosed purpose of the visit to Caregiver Demarto Acupan.
The LPA reviewed records of staff training and facility operations.
No citations issued during visit. 1 Technical Violation issued for violation of regulation that does not pose a risk to the health and safety of persons in care.
