Home Care Organization Evaluation Report

Enforcement Analyst (EA) Megan Vigil arrived at the residential business location for Senior Care Allies on November 19, 2025, to conduct the initial inspection. No one answered the door. EA Vigil attempted to contact the licensee, Dale Corpus, by calling the listed phone number and sent an email notifying him of her arrival and requesting a response.

Licensee Corpus later advised EA Vigil via email that he would like to close the business. EA Vigil instructed the licensee to submit a formal closure letter to the Department. The closure letter was dated November 24, 2025, and was received and processed by EA Vigil on November 25, 2025. The letter was signed and dated under penalty of perjury.

EA Vigil forwarded the closure letter to the assigned Licensing Analyst for completion of the closure process.
