Facility Evaluation Report

On 01/29/2025 at 1:50 PM, Licensing Program Analyst (LPA) A. Gomez arrived unannounced to conduct Required 1 Year Annual inspection. Upon arrival, LPA was greeted by Licensee/Administrator, Mona Eweda and explained the purpose of the visit. The facility’s fire clearance was approved for 6 Non-Ambulatory.

Due to the behaviors of the Licensee and safety the LPA was not able to conduct the Annual Inspection. LPA will return at a later date to complete the annual inspection and issue deficiencies.

The Following Deficiencies were observed:

Facility front door has a bolt lock with a combination on the inside

Licensee intentionally made false claims to LPA regarding individual at facility.

Due to Licensees hostile nature LPA will provide a copy of this report via certified mail
