COMPLAINT CONTROL NUMBER: 15-AS-20260413084301
Staff interfered with the ombudsman's ability to perform their duties
On 4/17/2026 at 2:00 PM, Licensing Program Analyst (LPA) A. Gomez arrived unannounced to conduct complaint investigation for the above allegation. LPA met with Caregiver, Edel Ann Manga and explained the purpose of the visit. Administrator Yanlin "Cynthia" Huang was unavailable and approved caregiver to sign off on report via telephone call.
During the complaint investigation, LPA spoke with the Administrator over the phone who acknowledged that they did not give the documents/information for a resident to the ombudsman upon request as of todays date however upon review of regulation it was determined that the action did not violate Title 22 regulation therefore the allegation is Unsubstanciated.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
