02/17/2022 and conducted by Evaluator Lizette Francisco

COMPLAINT CONTROL NUMBER: 15-AS-20220217133453

Facility has inappropriate locking device on facility doors.

On 2/25/2022 at 12:05 PM, Licensing Program Analyst (LPA) L. Francisco arrived unannounced to conduct complaint investigation for the above allegation. LPA met with Administrator Yanlin "Cynthia" Huang and explained the purpose of the visit.

During the complaint investigation, LPA conducted a tour of the facility. Based on information obtained, facility has inappropriate locking device on facility doors. LPA observed a latch above the front main entrance and R1's exit door.

Based on LPAs observationss, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations (Title 22, Division 6, Chapter 8), are being cited on the attached LIC 9099D.
