The inspector’s account

On 04/25/2025 at 2:40 PM, Licensing Program Analyst (LPA) A Gomez arrived unannounced to conduct a case management visit in regards to information provided to the department on 04/18/2025. LPA met with Administrator, Karla Sagastume and explained the purpose of the visit.

It was reported that the facility is attempting to be reimbursed for therapy that they paid for C1. LPA spoke with Administrator who states that they will provide all correspondences and contracts pertaining to this incident by 4/28/2025. LPA will review the documents and conduct an additional case management visit if required.

No deficiencies cited. Exit interview conducted and a copy of this report provided.
