Facility Evaluation Report
On 7/09/2026 at 1:45PM, Licensing Program Analyst (LPA) Carol Fowler arrived unannounced to conduct a Required - 1 Year inspection. LPA rang the doorbell a few times with no response. LPA called Administrator and there was no answer, LPA then texted Administrator. LPA waited outside of the facility and left at 2:25PM. Administrator texted LPA then called, LPA had left the facility. Administrator informed LPA that Administrator left her phone in the car. That's how she missed the call. An attempted visit was made and LPA left the facility around 2:25PM.
On 07/21/2026 at 11:30AM, Licensing Program Analyst (LPA) Carol Fowler conducted an unannounced annual 1-year required inspection. LPA met with Pia Webster, Administrator and explained the purpose of the visit. The facility’s fire clearance was approved for six (6) ambulatory clients.
LPA toured the facility including but not limited to bedrooms, bathroom, kitchen, common area, and back yard. The facility consists of four (4) total bedrooms and two (2) bathroom. All indoor passageways are kept free of obstruction. There are no bodies of water.
