Facility Evaluation Report

On 05/23/2025 at 11:00 AM, Licensing Program Analysts (LPAs), Y. Brown and L. Fontanilla arrived unannounced to conduct a health and safety check as a result of the department receiving a complaint with the control number (#15-AS-20250522110010.) LPAs met with Administrator Eleina L. Ridolfi and explained the purpose for the visit. The Administrator authorized Heather Payne, Clinical Care Director to sign the report.

LPAs observed clients sitting in common area, backyard, and dining area. During the health and safety check, LPAs toured the building with Clinical Care Director Heather Payne and Lead Floor Supervisor Priscella Saavedra. LPAs toured the common areas, bathrooms, bedrooms and outdoor area. LPAs observed and measured a comfortable water temperature at 115.3 degrees Fahrenheit.
