Facility Evaluation Report
On 04/15/2025 starting at 2:30 PM, Licensing Program Analyst (LPAs) David Doidge arrived unannounced to conduct a Required 1-Year Inspection. LPA met with Caregiver Abril Juarezand explained the purpose of the visit. Administrator Paul Scattolin was notified of the visit and arrived at 2:45 PM.
LPA toured facility including but not limited to bedrooms, bathrooms, and there is one ADU in the backyard. All outdoor and indoor passageways were kept free of obstruction. There are no bodies of water observed. A comfortable temperature of 72 degrees was maintained at the facility. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients; clients bathroom needs non-skid mats. Water temperature measured in a common bathroom was 111.8 degrees Fahrenheit. There was a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detector were in operating condition during visit. Fire extinguisher was last serviced on 01/08/2026. Emergency Disaster Plan was posted. First aid kit complete. Emergency disaster drills are conducted monthly, last one conducted on 03/36/23026.
LPA reviewed four (4) client records and five (5) staff records, all were complete.
Exit interview conducted and a copy of this report provided.
