Facility Evaluation Report
On 3/10/2026 at 10:45 am, Licensing Program Analyst (LPA) Y.Brown conducted an unannounced annual required inspection. LPA met with Community Programs Director Ember Avalos and explained the purpose of the visit.
The facility’s fire clearance was approved for seventy (70) clients. LPA toured the facility including but not limited to activity rooms, restrooms, kitchen, common area, and back yard. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ restroom was measured at 112.4 degrees Fahrenheit.
All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for clients. Smoke and carbon monoxide combination were observed during visit. Fire extinguisher last serviced on 6/18/2025. The Fire Drill was reviewed to be last conducted on 1/27/2026.
