Facility Evaluation Report
On 01/07/26 at 12:30PM, Licensing Program Analyst (LPA) Daisy Panlilio conducted an unannounced required annual inspection and met with administrator (ADM). LPA explained the purpose of the visit with ADM.
At 1PM, LPA toured the facility with ADM including but not limited to clients' activity room, sensory/relaxation room, changing room (1), office, bathrooms (2) and kitchen. During visit, LPA observed a COVID-19 screening station with visitor's log, box of gloves, face masks, hand sanitizer and no touch temperature probe for staff and visitors' use. LPA observed adequate lighting in the facility with room temperature at 73 deg F. Indoor passageways were observed free of obstruction. Infection control leader is the ADM. LPA observed locked storage room for cleaning supplies and disinfectants. LPA also observed another locked storage room where emergency supplies and diapers are kept. Smoke and carbon monoxide detectors were tested operational. First aid kits (2) were complete. LPA observed hot water temperature measured at 114 degrees F in the bathrooms. The Day Program does not handle Personal and Incidental (P&I) money, medications or serve food to the clients.
LPA observed two (2) bathrooms equipped with trash bins with lids opened by foot pedals, proper hand washing poster and soap dispensers. LPA observed 5 fire extinguishers fully charged located in the kitchen/common areas and last inspected on 10/23/25.
