The inspector’s account

On 07/28/2026 at 12:50 PM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct a 1-Year Annual Required Inspection . LPA met with Program Manager, Teanna Jerkins, and explained the purpose of the visit. The day program operates from 9:30 AM to 3:30 PM . There were thirty-one (31) participants in attendance and five (5) staff observed providing supervision and services during today's inspection. The facility has a fire clearance approved for a capacity of forty-five (45) ambulatory participants .

LPA toured the facility with Ms. Jerkins, including, but not limited to, the multiple activity rooms, kitchen, bathrooms, office space, and outdoor recreational area. Participants bring their own lunches and snacks. Emergency supplies, including an adequate supply of water and snacks were observed. The hot water temperature in the participants' bathrooms measured 110.8, and 105.5 degrees Fahrenheit . Centrally stored medications, toxic substances, and sharp objects were locked and inaccessible to participants. No bodies of water or fire safety hazards were observed.

The restrooms were maintained in a safe and sanitary operating condition, and adaptive equipment for individuals with physical disabilities was observed. Incontinent participants were observed to be clean and dry, and the facility was free of objectionable odors.
