On 09/22/2025 Licensing Program Analyst, (LPA) L. Holmes conducted a announced Collateral Case Management for complaint #15-AS-20250828161548 dated 08/28/25. LPA met with Lead Support Coordinator (S1) and explained the purpose of the visit.

LPA and S1 conducted and explained the importance of the interview for the complaint investigation with Client #1 (C1), and that all names would remain confidential. LPA interviewed Client #2 (C2), separately and explained the importance of the interview for the complaint investigation and it's confidentiality. LPA requested that S1 provide LPA with the ID/Emergency Contact information for Client #3 (C3).

Exit interview conducted and a copy of this report provided to Maci Spehard, Lead Support Coordinator.
